Use of non-invasive mechanical ventilation in two intensive care units in Bogotá in relation to international references

Abstract

Introduction: Non-Invasive Mechanical Ventilation (NIMV) provides mechanical ventilatory support without the use of an artificial airway and is widely used within Intensive Care Units. Objective: to characterize the use of NIV in two ICUs in the city of Bogotá in light of the international standards that support it. Materials and methods: descriptive, observational study, with a convenience sample aimed at the adult population within the Intensive Care Unit (ICU), with information collected from 31 patients who met the inclusion requirements and were subjected to the use of the NIV in a period between October and December 2015. Results: the management of patients undergoing NIV in these two ICUs in the city of Bogotá is linked to international guidelines referenced in world literature. Compliance with: indications, interfaces and establishment of the initial ventilatory parameters is highlighted, including the titration of the Positive End Expiratory Pressure (PEEP) of 8-12cmH2O to improve the oxygenation status and maintain functional residual capacity, the support pressure established between 10 to 12cmH2O to reduce respiratory work by the respiratory muscles, and the sensitivity level set at -2L/s, in order to reduce and avoid episodes of asynchronous patient ventilator, among others. Conclusions: more studies should be carried out on the management of NIV in Colombia, which allow professionals and institutions to evaluate and compare their management protocols and levels of adherence in relation to both national and international literature.
